No one has to deal either poor eyesight or healing loss (especially since they shoot guns constantly without protection) Nobody gets worms or other parasites from eating bad food. Or dies from dehydration due to diarrhea. Etc etc.

It’s almost like the point of these scenarios is fun escapism and isn’t about perfectly simulating an apocalyptic wasteland down to the most mundane and uninteresting parts.
